Installation:
unrar or unzip the usermaps folder with ALL Files in your COD4 directory.
C:/Program Files/Activision/Call of Duty 4 - Modern Warfare/
put the mp_dust2_classic.IWD only into the usermaps/mp_dust2_classic folder
not into the MOD Folder.
